Failure Analysis
Engineering Failure Analysis: Application of scientific methodology, knowledge and experience to investigate an engineering failure, incident, or accident, with the aim to identify the mechanism(s) and root cause(s) and provide recommendations on countermeasure(s) to prevent similar failure, incident or accident or minimize the failure rate.
Forensic Investigation
Forensic Investigation: Application of scientific methodology, knowledge and experience to investigate an engineering failure, incident or accident.
Apart from identifying the mechanism(s) and root cause(s) of a failure, the investigated case could be related to a litigation matter with multiple parties involved, such as the original equipment manufacturer, distributor, end-user, contractor, insurance companies and even government agencies. The investigator is often engaged as an expert witness to testify his findings or comment on other experts’ findings in a court trial.
Root Cause Analysis
Root Cause Analysis (RCA): Problem-solving method used for identifying the root causes of faults or problems. It can help define corrective, prevent incident/accident recurrence, and mitigate operational risk with an effective predictive maintenance solution. Additionally, it can provide great support to attention on how the safety management system "allowed” deficiencies to exist, rather than individual blame or equipment failure. Therefore, RCA can be a very useful method for learning from incidents/accidents and ensuring preventative measures being put in place to keep all relevant stakeholders safer in the future.
Fire Investigation
Fire investigation: Practice of examining sites of fires to determine their causes. In fire investigation, the main objective of an investigator is to determine where, how and why the fire occurred.
Expert Witness
“An expert witness is generally defined as someone with sufficient skill, knowledge, or experience in a given field so as to be capable of drawing inferences or reaching conclusions or opinions that an average person would not be competent to reach.” (NPFA 921).
Expert Technical Services
Failure and forensic investigation are normally performed after an incident occurs, so to that extent, it is reactive action rather than a proactive one. It will be more appreciated to provide a proactive solution which can eliminate faults/incidents before they have a chance to appear. Drillconn’s experts have built up extensive experience and expertise in most of the industrial sectors, and can provide proactive expert technical services, e.g., risk-based design review, inspection, troubleshooting, condition assessment, etc.
